online formsWe encourage you to fill out the necessary forms prior to your appointment or drop-off date, to expedite your pet’s check-in. These forms are also available at our reception desk.

  • New Client Form—Please submit if this will be your first visit to our hospital or boarding kennel.
  • Hospital Admit/Surgery Consent Form—Please submit each time you will be admitting your pet to our hospital for services or for surgery. If your pet is having a dental service along with another procedure (for example, annual exam, neuter, etc.), you only need to submit a Dental Consent Form, but please note any additional services that will be done.
  • Dental Consent Form—Please submit each time you are admitting your pet for a professional teeth cleaning.
  • Boarding/Grooming Consent Form—Please submit each time you arrange for boarding or grooming at our Boarding Kennel or Feline Practice Boarding.
  • Boarding/Grooming Release Form—Please submit if this will be your pet’s first time boarding or grooming at our Boarding Kennel or Feline Practice Boarding or to update a previous release form submitted.
  • Medical History/Risk Assessment Form—This form helps us have a better understanding of your pet’s current medical conditions and concerns you may be having. Please complete this form in addition to your consent form.
  • Consent for Transportation to Another Facility—Please submit when your pet is being transported to another veterinary facility or emergency hospital.
  • Concierge Services Transportation Consent—Please submit when your pet is being picked up and/or dropped off by our Pet Concierge.
